Sarah is a board-certified Adult Nurse Practitioner with more than two decades of experience caring for adults across every stage of life. Her work has spanned primary care, chronic disease management, and the long-arc, relationship-based care that builds slowly over years of visits.

She started Foundation Health Group to practice the kind of medicine she trained to deliver. Insurance billing, twelve-patient mornings, and the relentless hum of administrative work had compressed primary care down to a fifteen-minute exchange, often with a different provider each time. Direct primary care, by telehealth, made it possible to put that practice back together: fewer patients, longer visits, direct messaging, and an unhurried way of working that lets the relationship do its quiet work.

How she works

Sarah reads your records in advance so the visit gets spent on you. She asks what brought you here, what you've tried, what you're worried about, and what you actually want from your health. From there, together, you build a plan that fits your life and that you'll actually follow.

The plan stays alive. Sarah revisits it over months and years, adjusts what isn't working, and watches for patterns that only become visible when one provider stays with you through the long arc.
